Encouraging your people to fill out Form Cards in Church Connect will cut down on the workload for your church leadership and save money on printing costs!
After creating a new Form Card (or by clicking on a Form Card you've already created), designate the ChurchTrac user who will receive emails containing the responses from each form submission.
Next, click "Form Questions" button and begin adding your questions.
Proceed in creating your first field. You can select from multiple field types and even add a description. Continue adding more fields. Once you've finished, you can customize the form with field requirements, visibility, order & more.
To view your Form Card responses at once, click the "Form Responses" tab on your Church Connect Screen. You can either view all responses to all forms or select a form on the left and view responses to only that form. Form card responses can also be printed out or exported to CSV, which allows you to store the data elsewhere.

If you have a Twilio account integrated with your ChurchTrac account, you can set up auto-replies so that your people can text a keyword to your church and receive a text in response. Link your form cards in these texts to make it easier for everyone to navigate to them.

You can set a form field to show or not show to the user based on whether or not that person is logged in. When logged in, we'll automatically inject the user's Name and Email into the form submission for you, so you only need to ask for this information when the member has NOT logged in.
